NRC INSPECTION

REPORT

NO. 50-528/529/530/91-19

This refers to the inspection

conducted

by Messrs.

D. Coe, J.

Ringwald,

J.

Sloan,

H. Wong,

and

P.

Galon of this office on May 12 through June

15,

1991.

The inspection

included"a review of activities authorized for your

Palo Verde facility.

At the conclusion of the inspection,

the findings were

discussed

with those

members of your staff identified in the enclosed

report.

Areas

examined during the inspection

are identified in the report.

Within

these

are'as,

the inspection consisted

of selective

examinations

of procedures

and representative

records,

inter views with personnel,

and observation of

activities in progress.

Based

on the results of this inspection, certain of your activities appeared

to be in violation of

NRC requirements,

as specified in the enclosed

Notice of

Violation (Notice).

The three

examples

included in Violation A are of concern

because

each of these

examples,

and several

other issues

also addressed

in this

report, relate to the improper performance of fundamental

work practices.

The

violation regarding the unbolting of the incorrect reactor coolant

pump seal

injection filter which resulted in a small reactor coolant leak is of

particular concern

because it represents

the failure of maintenance

personnel

to adhere

to several

administrative barrier s in your work control

system to

prevent

such events.

In addition,

a cursory work order revi'ew by the

gC

inspector resulted

in his failure to recognize,

question,

and prevent this

event from occurring.

These worker performance

errors

appear

to warrant

prompt corrective action.

Also, we request that this matter

be included

among

the topics to

b'e discussed

in the upcoming senior

management

meeting.

You are required to respond

to this letter and should follow the instructions

specified in the enclosed Notice'hen

preparing your response.

In your

response,

you should

document

the specific actions

taken

and any additional

actions

you plan to prevent recurrence.

After reviewing your response

to this

Notice, including your proposed

corrective actions

and the results'f future

inspections,

the

NRC will determine

whether further

NRC enforcement

action is

necessary

to ensure

compliance with NRC regulatory requirements.
