Palo Verde Nuclear Generating Station Unit 3 Notification of Unusual Event Docket No. 50-530 License No. NPF-74 Special Report No. 3-SR-91-005 This Special Report is being provided pursuant to Emergency Plan Implementing Procedure (EPIP)-03, "Notification of Unusual Event Implementing Actions" to report a condition requiring the declaration of a Notification of Unusual Event for Palo Verde Unit' on July 12, 1991, between the hours of 1741 and 2045 MST.

On July 12, 1991, at approximately 1741 MST, Palo Verde Unit 3 was operating at 100 percent power in Mode 1 (POWER OPERATION) when a reactor operator, performing a post-maintenance reactor coolant system water inventory balance, determined that reactor coolant system UNIDENTIFIED LEAKAGE was approximately 1.322 gallons per minute (gpm) ~ The leakage exceeded Technical Specification (TS) 3.4.5.2 Limiting Condition for Operation (LCO) for reactor coolant system leakage limit of one (1) gallon per minute UNIDENTIFIED LEAKAGE. Emergency Plan Implementing Procedures require the declaration of a Notification of Unusual Event for exceeding TS leakage limits. TS 3.4.5.2 ACTION (b) requires a reduction in the leakage rate to within limits within four (4) hours or be in at least HOT STANDBY within the next six (6) hours and in COLD SHUTDOWN within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

Unit 3 personnel investigated and determined that the leakage was attributed to seat leakage from a reactor coolant pump seal injection heat exchanger drain valve. The leakage was directed to the non-Engineered Safety Feature sump located in the Auxiliary Building. At approximately 1830 MST, the drain valve was seated. At approximately 2045 MST, on July 12, 1991, following the performance of the reactor coolant system water inventory balance, Control Room personnel verified that reactor coolant system UNIDENTIFIED LEAKAGE had been reduced to within limits and TS 3.4.5.2 ACTION (b) was exited. In addition, the condition requiring the declaration of a Notification of Unusual Event was terminated. Unit 3 had remained in the TS ACTION statement for approximately three hours.

Initially, PVNGS management interpreted Emergency Plan Implementing Procedure requirements, which classify UNIDENTIFIED LEAKAGE exceeding TS 3.4.5.2 reactor coolant system leakage LCOs as a Notification of Unusual Event, to apply to a situation where the TS ACTION could not be met [i.e., UNIDENTIFIED LEAKAGE rate could not be reduced to within limits within four (4) hours]. Under this interpretation, management determined that the event did not require a declaration of a Notification of Unusual Event.

Based on a subsequent review of the event, PVNGS management determined that a Notification of Unusual Event was required in accordance with the Emergency Plan Implementing Procedure. At approximately 1327 MST on July 15, 1991, the Nuclear Regulatory Commission (NRC) Operations Center was notified of the

UNIDENTIFIED LEAKAGE exceeding TS leakage limit): By approximately 1430 MST, on July 15, 1991, the appropriate state and local agencies were notified.

Further investigation into the circumstances surrounding this event is being performed in accordance with the PVNGS Incident Investigation Program.
