discussion of our findings held by the inspectors with members of the

Arizona Public Service

Company staff at the conclusion of the inspection.

Areas

examined during this inspection

are described

in the enclosed

inspection report.

Mithin these

areas,

the inspection consisted of

selective

examinations of procedures

and representative

records,

interviews with personnel,

and observations

by the inspectors.

This report discusses

several

instances

in which operations

personnel

performance contributed to component operability problems or highlighted

areas

in which control

room practices

needed

improvement.

One of these

involved the manual mis-operation of an Atmospheric

Dump Valve (ADV)

despite

the significance of manual

ADV operation

as evidenced

in the

March 3, 1989, reactor trip event

and the several restart

commitments

associated

with manual

ADV operation.

Although the direct safety

significance of this event

was minimal, a violation is being cited for

thss matter

due to the repetitive nature of this issue.

Another instance

involved the inadvertent rendering of Safety Injection Tanks technically

inoperable

by providing power to the vent valves.

These

events

and

others

described

in the inspection report reflect a need for greater

management

and supervisory attention to operator

performance.

Based

on the results of this inspection, it appears

that several

of your

activities were not conducted in full compliance with NRC requirements,

as set forth in the Notice of Violation, enclosed

herewith as Appendix A.

Your response

to this Notice is to be submitted in accordance

with the

rovisions of 10 CFR Part 2.201

as stated

in Appendix A, Notice of
