This refers to the special

inspection

conducted

by Mr. L. F. Miller, Jr. of

this office on July 16 - August 7,

1990 of activities authorized

by

NRC

Operating Licenses

NPF-41,

NPF-51,

and NPF-74,

and to the discussion of the

findings held with you and other

members of youI staff at the conclusion

of. the

inspection.

Me were concerned that the degree of management

attention

and associated

management

controls were inadequate

to assure

that the medical records

and

operator qualifications were being maintained.

However,

we are encouraged

that

the programs

which you have developed recently, if properly implemented,

appear

to be appropriate to correct the deficiencies

which you and the

NRC have

ident ified.

This inspection

was to review the administrative controls for assuring

the

maintenance

of operator

and senior operator licenses.

Specific areas

examined

during this inspection

are described

in the enclosed

inspection report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation

by the

inspectors.

Based

on the results of the inspection, it appears

that your program to ensure

that all licensed operators

were medically qualified at all times was not

fully effective.

Specifically, the inspection identified inaccurate

certifications that

a complete pre-licensing medical

exam had been performed,

incomplete or late medical

exams of 'operators,

and medically unqualified

operators

who had stood or were standing watch without required notification of

the

NRC.

Enforcement actions relative to the findings discussed

in the

enclosed report will be the subject of separate

correspondence.

Some of these

examples of the above mentioned deficiencies

were identified

by this inspection after

we notified you that

a complete review of medical

records

was needed.

Specifically, you were notified by the

NRC in November

1989, again in June

1990,

and finally, in July 1990, that this was

a program

which appeared

weak.

The reviews which you subsequently

conducted

were not

fully comprehensive,

as discussed

in this report.

in the near

future.
