An NRC licensed operator requalification simulator operating retake examination was administered to one Senior Reactor Operator (SRO) at th' Palo Verde Nuclear Generating Station (PVNGS), Units 1, 2, and 3 on March 20, 1990.

Results:

Based on the performance of the Senior Reactor Operator (SRO) during the simulator operating retake examination, the SRO was evaluated as satisfactory by the NRC Chief Examiner.

The operator's file will be updated to reflect the satisfactory performance.

Examination The examination was conducted to reexamine a Senior Reactor Operator in the simulator operations portion of the Licensed Operator Requalification Program evaluation.

The operator had been previously evaluated as unsatisfactory, by both the licensee and the NRC, in the simulator operations portion of the NRC evaluation of PVNGS's Licensed Operator Requalification Program.

The results of the previous examination were documented in Examination Report 50-528/OL-89-02.

The licensed operator requalification simulator operating retake examination was administered in accordance with Examiner Standard ES-601, Revision 5, "Administration of NRC Requalification Program Evaluations".

Examination Develo ment The simulator scenarios and licensee supplied reference material were reviewed by the chief examiner in accordance with ES-601.

Fifteen scenarios were originally submitted by the licensee for the administration of the retake examination.

The fifteen simulator scenarios were the same scenarios that were used in the October, November 1989 NRC evaluation of the PVNGS licensed operator requalification program.

Therefore, the lead facility representative for this requalification retake examination and the NRC Chief Examiner agreed that a minimum of three new scenarios needed to be developed in order to ensure a valid and secure retake examination.

The licensee representative developed three new simulator scenarios for the examination.

The NRC Chief Examiner reviewed the scenarios in accordance with ES-601 and accepted the new scenarios for the administration of the simulator requalification retake examination.

Two of the three new scenarios developed for the examination were used during the administration of the examination.

The simulator scenarios for this examination were developed on the PVNGS revision 15 computer model "platters."

The PVNGS personnel developing the scenarios for the examination were able to pre-run and debug the scenarios, with the NRC Chief Examiner prior to the examination, to minimize the probability of anomalous events occurring during the administration of the examination.

Modeling and hardware simulator problems

observed, by the NRC, during the administration of the examination are noted in Enclosure 4

of the cover letter to this report.

All of the licensee personnel who participated in the development, review, and administration of the examination scenarios signed security agreements, as required by ES-601.

On March 21, 1990, the NRC Chief Examiner met with the licensee staff to discuss the examination.

PALO VERDE NUCLEAR GENERATING STATION MARCH 20, 1990 RE(UALIFICATION RETAKE EXAMINATION SIMULATION FACILITY FIDELITY REPORT The simulator scenarios provided for this examination were prevalidated and prerun by licensee representatives and the NRC Chief Examiner.

The scenarios were developed and run using the PVNGS revision 15 simulator model program platters.

The prevalidation and prerunning of the sc'enarios minimized the anomalous events occurring during the administration of the examination.

Modeling problems noted during the administration of the examination.

ESF switch gear room normal air handling unit fan N indicated that the unit was running when the controller switch, HJN-HS139, was in the stop position.

Control Building essential normal air supply switch, HJN-HS54 for AFU Fan AOl indicated both stop and run indication while the unit was running.

The mode select switch for the CEDMCS did not proper ly position to the proper mode.

The switch would position in between two modes.

This was a facility identified hardware/maintenance problem.

No anomalous simulator events occurred during the administration of the requalification simulator retake examination.
