Palo Verde Nuclear Generating Station (PVNGS) Unit 3 Restart Docket No. STN 50-530 (License No. NPF-74)

File 89-056-026 This letter confirms compliance with the agreed upon course of action for the restart of PVNGS Unit 3 as outlined in Reference (1). With the modification of the item discussed in the next paragraph, this submittal also confirms completion of the items contained in the PVNGS Unit 3 Restart List which was provided to you as an attachment to Reference (2). The specific actions taken to address those items have been documented in individual closure packages previously provided to the NRC Senior Resident Inspector.

Record 91 in the Unit 3 Restart List provides for an inspection of all Reactor Coolant Pump seals to determine further the root cause of failure of the seals during the Unit 3 event on March 3, 1989. An engineering investigation of the 1B Reactor Coolant Pump seals determined the root cause of failure to be a thermal transient, caused by the flow of hot reactor coolant system water near normal control bleedoff flowrates with no interstage cooling or seal injection, which damaged all three seal stages. Since the root cause could be determined from the evaluation of the seals of one pump and was not associated with the design, construction, or assembly of the seals, inspection of the remaining pump seals would not contribute to a further determination of root cause. Therefore, as previously discussed with members of your staff, the objective of record 91 was accomplished in a manner different from that stated. All four (4) RCP seals have been replaced.

Although not a specific restart action as specified in Reference (1), APS has identified concerns for the operability of the Unit 3 Essential Turbine Driven Auxiliary Feedwater Pump, During testing of this pump following the refueling outage, abnormal suction and discharge pressure oscillations were noted. Further testing and analysis has determined the cause as discussed with NRC Region V on 9001290244 S91224 PDR AGOCK 05000530 P PDC

John B. Martin Page 2 December 22, 1989. A modification to correct this condition has been implemented and successfully tested.

As discussed with the NRC in Walnut Creek on October 10, 1989, a formal Unit 3 Restart Oversight Program was implemented under the direction of the Management Review Committee. The Committee has evaluated the actions taken in accordance with the Restart Program, concluded that Unit 3 is ready for restart and so advised me. I have reviewed the status of preparations to restart Unit 3 and have so determined that Unit 3 is ready for restart.

This letter, together with APS's presentations to you and your staff, stand as APS's certification of compliance with your confirmatory action letter of June 28, 1989.
