J. Ball, M. Davis,

Foley of the
NRC Office of Nuclear Reactor Regulation,
on April 27
through ilune 11,
1989, of activities authorized
by
NRC 'License
Nos.
and NPF-74,
and tn the discussion
of our findings held by
the inspectors
with members of the Arizona Nuclear
Power Project staff at
the conclusion of the inspection.
Areas
examined durinq this inspection
are described
in the enclosed
inspection report.
k'ithin these
areas,
the inspection consisted
of
selective
examinations
o
procedures
and representative
records,
interviews with personnel,
and observations
by the inspectors.
Although no deviations or violations of NRC requirements
were identified
within the scope
o
this inspection,
the results of our reviews in
several
areas
continue to cause
us to have concern with your performance
of technical
work.
Specific examples
are
as follows:
Your engineerin'q staff recen.ly reviewed
a technical
issue
regarding
emergency
diesel
generator
excessive
flow check valves,
and concluded that the issue required
no action.
Our independent
review found your conclusion to be technically unsupported
and in
need of further engineering
work to resolve.
Our assessment
of your initial engineering
review of the adequacy
of emergency lighting at Unit 2 found your review to be incomplete
and lacking in scope
and depth,
as evidenced
by the additional
lighting deficiencies identified by our inspector.
We are
particularly disturbed that our inspector identified one
room at
Unit 2 with five of six normal lighting fixtures with burned out
light bulbs, in spite of the sensitivity to lighting problems that
was created
by the Unit 3 event
on March 3,
1989.

Regarding your efforts to ensure reliable communications
between
the control
room and operators
out in the units, 'although
we
recognize that your are attempting to address
the issue, it is
clear that your engineering
actions to date
have not resolved
the
issue.
We question
whether
a sufficient level of engineering
attention is being directed at this problem to promptly and
adequately
resolve it.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the
enclosure will be placed in the
NRC Public Document
Room.
