This letter refers to the routine inspection
conducted
by Messrs.
M.
Cillis,
G. Cicotte
and
L.
Carson of this office on June 5-15,
1989,
June
26-30,
1989,
and telephone
conversations
on July 5, 1989, of activities authorized
by NRC
License
Nos.
and to the discussions
of our findings
held by Mr. Cillis with Messrs
P.
W.
Hughes,
J.
E. Kirby and other
members
of
your staff at the conclusion of the inspection period
on June
15,
1989 and
on
June
30,
1989.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that several
of your
activities were not conducted
in full compliance with NRC requirements
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.
The enclosed
inspection report is worthy of your careful
review.
The
observations
described
in paragraphs
2.G and 5.
appear to have
a. common
denominator.
They are:
inattention to detail,
weak procedures,
failure of
personnel
to comply with written procedures
and/or instructions,
and
inadequate
review of surveillance
test results.
Additionally, we have
particular concerns
regarding the timeliness with which corrective actions
are
taken in addressing
self-identified problems.
This concern
was brought to
your attention
on several
occasions
in the past.
While the
NRC enforcement
policy encourages
licensees
to identify and correct deficiencies,
we also
expect corrective actions to be implemented in a timely manner
and to be
lasting and effective.
We hope that your post-inspection critique will focus
on the root causes
for these
observations
such that your corrective actions
will minimize the potential for a recurrence
of similar observations.
We will
monitor your timely actions to correct this situation during a subsequent
inspection.
Your response
to this Notice is to be submitted in accordance
with the
provisions of 10 CFR 2.201
as stated
in Appendix A, Notice of Violation (NOV).
In accordance
with 10 CFR 2. 790(a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
