The purpose of this letter is to confirm our understanding of the actions you intend to take, 1') at Units 1 and 2 regarding the Unit 3 reactor trip which occurred on March 3, 1989',

and 2) at Uni.t,l regarding the reactor trip which occurred on March 5, 1989.

Based on the discussions

'held between Mr. A. E.

Chaffee, of my staff, and you, it is our understanding that your actions will include,, but will not necessarily be limited to, the,following:

1.

You will incorporate the central lessons learned from the'March 3, 1989, Unusual Event at Palo Verde 3 into your activities at Unit 1 prior to restart of Unit 1'.

This wil] include confirmation that the atmospheric dump valves, emergency lighting, and steam bypass control system are operable.

Other lessons learned will be incorporated prior to restart to the extent they are reasonably understood.

'ikewise, lessons learned from the Unit 3 event will be applied to Unit 2 expeditiously, commensurate with the safety significance of the deficiency.

You will conduct a thorough investigation and obtain a full understanding of the Unit 1 March 5, 1989, reactor trip and define the pre-restart and post-restart corrective actions needed as a result of this event.

4, You will develop a plan. for investigation of the Unit 1 NAN-S02A circuit breaker failure which occurred on March 5, 1989, and present it to the NRC prior to commencing troubleshooting.

This plan will include provisions for prior notice to the. NRC of your intent to release the circuit breaker from quarantine for troubleshooting or other essential.

This notice will provide sufficient time for NRC representatives to arrange-for review of your specific troubleshooting procedures and to be present at the job site.

None of the above shall preclude those'ANPP actions needed to maintain and ensure plant safety.

You will brief the NRC upon completion of your investigation and will not restart (enter Mode 2) the Palo, Verde Unit 1 facility prior to receiving NRC concurrence.
