Examination Report On November 15, 1988 and during the period of November 28 through December 8, 1988 the NRC administered examinations to eleven Reactor Operator and thirteen Senior Reactor Operator candidates, who had applied to operate your Arizona Nuclear Power Project's, PVNGS Units 1, 2, and 3.
All of these candidates passed their examinations and were subsequently issued a license.
The simulator continues to display fidelity problems to the extent that comprehensive NRC operating examinations can not 'be administered without a
thorough prevalidation of the intended scenario evolutions and malfunctions.
Furthermore, during one of the operating examinations a safety related procedure for emergency boration was found missing from its procedure locker by the Unit 3 remote shut down panel.
Technical errors were also identified within this procedure during the course of the examination.
These issues were discussed with the Senior Resident, and the members of your staff identified in the enclosed report.
In accordance with 10 CFR 2.790(a),
a copy of this letter and enclosures (1) and (2) wi 11 be placed in the NRC's public document room.
