G. Fiorelli of this office on September
18 through
November 5, 1988, of
activities authorized
by
NRC License
Nos.
NPF"41,
and
to the discussion
of our findings held with members of your staff at the
conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection report.
Within these
areas,
the inspection consisted of
selective
examinations
of procedures
and representative
records,
interviews with personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that one of your
activities was not conducted
in full compliance with NRC requirements,
as
set forth in the Notice of Violation, enclosed
herewith
as Appendix A.
Your response
to this Notice is to be submitted in accordance
with the
provisions of 10 CFR 2.201 as stated
in Appendix A, Notice of Violation.
Additionally, a number of observations
made
by the inspectors
indicate
that your personnel
are not consistently
adhering to procedural
requirements.
We consider this observation to be directly related to the
degree to which management
expresses
their expectations
on
how work is to
be por ormed.
We also want to draw your attention to the event at Unit
1, wherein
a chemist apparently failed to properly align a sampling
system
when removing
a clearance
from that system.
We are concerned that
this event is very similar to the valve alignment problem with the Post
Accident Sampling System,
which occur red at Unit 2 in November,
1987,
and
that your corrective actions for that event did not prevent this most
recent problem.

Your attention is also, drawn to paragraph
8 of the attached
report,
wherein the circumstances
surrounding the occurrence
of a small fire in
the Unit 1 Radwaste
Building is discussed.
In addition to your response
to the Notice of Violation, please
provide your written assessment
of the
adequacy
of your procedure
governing the control of hot work
authorization permits.
Paragraph
12.c of the attached
report discusses
your development of an
Incident Investigation
Program.
Please
provide
us with a written
description of this program
and your schedule for implementation.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the
enclosures will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and
Budget,
as required
by the Paperwork Reduction Act of 1980,
PL 96-511.
