NRC Evaluation of the Palo Verde Nuclear Generating Station Units 1, 2, 3 Licensed Operator Requalification Program During the weeks of October 10 - 14 and October 24 - 28, 1988, the NRC conducted an evaluation of the Palo Verde Nuclear Generating Station Units 1, 2, 3 Licensed Operator Requalification Program.

The evaluation was based on written and operating examinations in accordance with NUREG 1021, Examiner Standards, ES-601, "Administration of NRC Requalification Program Evaluations",

dated October 1, 1988.

The purpose of the evaluation was to (1) determine the effectiveness of the Palo Verde Nuclear Generating Station (PVNGS) Licensed Operator Requalification Program, and (2) to evaluate individual operator s for renewal of six year term licenses.

Details and findings of the evaluation are contained in the Examination Report, which is enclosure 1 of this letter.

As noted in ES-601 the program may only be evaluated as Satisfactory or Unsatisfactory.

Based on the NRC evaluation, as conducted in accordance with the provisions of ES-601, the PVNGS licensed operator requalification program is SATISFACTORY.

One of the four crews observed during the simulator portion of the examination was evaluated as unsatisfactory by the facility and NRC evaluation teams.

One of the seven Reactor Operators observed during the simulator examination was evaluated as unsatisfactory by the facility and NRC evaluation teams.

The nine Senior Reactor Operators and six of the seven Reactor Operators observed during the simulator examination were evaluated by the facility and the NRC as satisfactory.

All sixteen of the licensed operators evaluated during the walk through portion of the examination were evaluated as satisfactory by both the NRC and facility evaluation teams.

Mr. D. Karner December 8, 1988 Nine of the nine SROs and seven of the seven ROs passed the written examination as graded by the NRC evaluation team.

Nine of the nine SROs and six of the seven ROs passed the written portion of the examination as independently graded by the facility evaluation team.

The Reactor Operator that did not pass the written examination was the same operator that was evaluated as unsatisfactory in the simulator portion of the examination.

It is a requirement of 10 CFR 55.57(b)(2)(iv) that all applicants for license renewal must pass an NRC administered written and operating test during the term of a six year license prior to the renewal of the operating license.

The recurring problem of simulation facility modeling and simulator fidelity was again noted in the preparation and administration of the licensed operator requalification program evaluation examination.

The problems noted during this evaluation are tabulated in enclosure (1)

(Examination Report) of this letter.

You are requested to address the simulator problems fr'om the points of, (1) preventing negative training of the operators, and (2) the ability to train operators for multi-unit licenses on a simulation facility that is not capable of being programmed to model

.the three different core cycles at the Palo Verde Nuclear Generating site.

In accordance with 10 CFR 2.790 of the Commissions regulations, a copy of this letter and enclosure 1, Examination Report, will be placed in the NRC Public Document Room.
