PALO VERDE NUCLEAR GENERATING STATION UNIT 1 Inoperable Seismic Monitoring Instrumentation License No. NPF-41 Docket No. STN 50-528 Special Report 1-SR-88-006 This Special Report is being submitted pursuant to Technical Specification 3.3.3.3 ACTION "a" and Technical Specification 6.9.2.

At approximately 0857 MST on August 17, 1988, Palo Verde Unit 1 was in Mode 3 (HOT STANDBY) with Reactor Coolant System (RCS) cold leg temperature at approximately 565 degrees F and RCS pressure at 2250 psia when the 30 day ACTION Statement for operability of the Seismic Monitoring System was exceeded. The Seismic Monitoring System had been declared inoperable at 0857 HST on July 18, 1988 due to spurious alarms.

Troubleshooting in accordance with an authorized work document identified the cause of the spurious alarms to be the trigger card (setpoint) for seismic accelerometer JSHNXT-0001 (Tendon Galley Floor, 55 foot level). The trigger card was found to be out of calibration and was subsequently recalibrated.

t Upon satisfactory performance of 36ST-1SM02 (Seismic Honitoring System Calibration Test) the Seismic Monitoring System was returned to operable status at 0537 HST on August 18, 1988. The Seismic Monitoring System was inoperable for approximately 31 days.
