Executive Vice President Gentlemen:
The purpose of this letter is to confirm our understanding of the actions you intend to take regarding the continued operation of Palo Verde Unit 2 and your evaluation and analysis of the circumstances surrounding the introduction of nonconservative information into the Channel B Core Protection Calculator.
Based on the 'discussions between Messers.
Dennis Kirsch of my staff, Gary Holahan, of NRR, and J.
G. Haynes, of your staff, it is our understanding that your actions will include, but not be limited to, the following:
1.
Complete your evaluation and analysis of the circumstances surrounding the introduction of nonconservative information into Unit 2 Channel B
Core Protection Calculator (CPC).
2.
Verify that the inputs to and operation of all Unit 2 CPCs are correct at 70% power. If any are found to be incorrect, power will not be increased above 70% until NRC and ANPP have agreed that the power can be safely increased.
3.
Verify the operation of Unit 1 CPC channels are acceptable.
4.
Within 30 days, submit a report to Region V, with a copy to NRR, which includes the results of your evaluation and analysis, root cause identified, and specifying the actions taken or to be taken which will preclude recurrence of the conditions or actions that resulted in the errors in the CPC software We understand that a problem with CPC Channel B was identified prior to going critical and Channel B has been in bypass, or tripped, during the investigation of the problem and, therefore, was not relied upon for safe operation of Unit 2.
