This refers to the inspection
conducted
by Messrs.
T. Polich, J. Ball,
G. Fiorelli, and J. Melfi, of this office, on January
17 through March 5,
1988, of activities authorized
by
NRC License
Nos.
and to the discussion of our findings held with members of your staff at the
conclusion of the inspection.
Areas
examined during this inspection are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that several
of your
activities were not conducted
in full compliance with NRC requirements.
Although these violations were identified by your staff, in each
case multiple
opportunities existed for your staff to take action to prevent the violation
prior to the violation occurring.
The recent
increased
frequency of these
type events
appears
to be indicative of your personnel
not fully understanding
the level of attention to detail
expected
by your managers.
Enforcement
action concerning
these violations will be the subject of separate
correspondence.
Your attention is also directed to paragraphs
7 and 10 of the enclosed
report.
As discussed
at our management
meeting
on February
29, 1988,
we are concerned
with the poor foreign material
exclusion practices
which led up to the stuck
control element
assembly
event.
Our concern
has since incr eased
in learning
that the failure of a multi-stud tensioner caster,
which resulted in the
release
of numerous ball bearings to the containment floor, was not brought to
your attention at the time of the caster failure, or during your initial
review of the stuck control element
assembly.
Paragraph
10 describes
our observation that operations
personnel
in the Unit 2
control
room were not resetting 'cleared annunciators
in a timely manner.

practice
has apparently
developed 'in part due to the relatively large
number
of inappropriately lighted annunciators.
As we have discussed
in several
meetings,
the reduction of these
inappropriately lighted annunciators
appears
to be an important step
towards ensuring
a prompt, thorough
response
to valid
alarms.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosure
will be placed in the
NRC Public Document
Room.
