The
NRC Systematic
Assessment
of Licensee
Performance
(SALP) Board has
completed its periodi'c evaluation of the performance of the subject
facilities.
The Palo Verde facilities were evaluated for the period
October 1,
1985 - September
30,
1986.
The results of the evaluation
are
documented
in the enclosed
SALP Board Assessment.
Me wish to discuss
the
results of this evaluation at the management
meeting tentatively scheduled for
late January,
1987.
The performance of your Palo Verde facilities was evaluated in the functional
areas of plant operations,
radiological controls, maintenance,
surveillance,
fire protection,
emergency
preparedness,
security
and safeguards,
outages,
quality programs
and administrative controls affecting quality, licensing
activities, training and qualification effectiveness,
containment
and
safety-related
structures
and major steel
supports,
piping systems
and
supports,
safety-related
components-mechanical,
auxiliary systems,
electrical
equipment
and cables,
instrumentation,
preoperational
testing
and startup
testing.
The
SALP Board's evaluation of your performance
in these functional
areas
is contained in the
SALP Board Assessment
which is enclosed with this
letter.
Overall, we find that your performance of licensed activities at the Palo
Verde Site was considered
to be satisfactory during this assessment
period.
However, there are several
areas of concern to which we wish to direct your
attention.
The most critic'al management
task at this point is to ensure that the proper
working atmosphere
is established
at Palo Verde.
This is an especially
important stage
in the plant life,and attitudes
formed
now will persist for
years.
A major challenge is to instill a questioning attitude in your
personnel
towards plant activities
and plant problems.
You should require
plant anomalies
and failures
be reviewed in detail
so that the cause of
failures
may be clearly determined
and promptly corrected.
Although progress
was made,
several
events
which occurred during this
SALP period indicate more
improvement in this area is needed.

Your facility record with regard to the number of plant trips, personnel
errors,
is clearly in need of improvement.
Additionally, the number of continuously lighted annunciators
and backlogged
maintenance
items appears
excessive.
Although progress
has
been. made in
these
areas,
more emphasis
is needed.
Finally, your performance in the area of security
and safeguards
was evaluated
as
a Category 3.
Although improved performance
in this area
was noted toward
the end of the assessment
period, continued additional attention
appears
appropriate to correct the weaknesses
identified.
We recognize the initiatives you have taken or have planned to address
the
concerns
discussed
above.
In several
areas
an improving trend
has
been
'observed.
Nonetheless,
we intend to closely monitor your performance
in
conjunction with considering your request for an operating license for Unit 3.
For the area of security
and safeguards,
please
provide
a written response
to
this office within 30 days of the date of this letter informing us of your
plans to achieve
improvement in this functional area.
In accordance
with Section 2.790 of th'e NRC's "Rules of Practice,"
Part 2,
Title 10,
Code of Federal
Regulations,
a copy of this letter and the
Report will be placed in the NRC's Public Document
Room,
as will any comments
you may wish to submit to
NRC regarding the content of the
SALP Report.
Should you have
