Dear Mr. Van Brunt,

Jr.:

As you know, during the operations/maintenance inspection conducted December 1-12, 1986, at your Palo Verde facility, samples of, safety-'elated fastener materials were obtained for subsequent chemical analysis and mechanical testing.

The results of this testing are enclosed.

The bolts designated as 6177-1 through 6177-14 in the report are the samples taken from the Palo Verde facility.

Your attention is directed to the bolts designated as 6177-5 through 6177-9.

The head markings on these bolts indicated they were SAE J429 Grade 8 bolts; however, the bolts failed to meet the chemical requirements of the SAE J429 specification (i.e., the five-bolts were found to be Grade 8.2).

We understand from discussions between Bill Craig of your staff and Ed Baker of the Vendor Inspection Branch that you were informally notified of the results of the testing by Region V staff and have taken some actions on this matter.

We apologize for the delay in formally transmitting the test results.

Please provide us within 30 days of the date of this letter, a written statement containing:

(1)

A description of steps that have been or will be taken to address this problem including a description of the intended application for these fasteners; (2)

A description of steps that have been or will be taken to prevent recurrence; and 1

(3)

The dates your corrective actions and preventive measures were or will be completed.

In addition, please identify the supplier(s)/manufacturer(s) who furnished the five bolts and also the supplier(s)/manufacturer(s) of fasteners who are currently on the APS Approved Vendor List.
