Request for Information - Palo Verde CEA Shroud Assembly In order for the staff to complete its materials review of the modifications made to correct the problems encountered with the Palo Verde CEA shroud assembly during hot functional testing, we require that you submit additional information.
Specifically, we request that you provide a copy of the following documents, relating to the materials, procedures and fabrication drawings, applied in the construction of the CEA shroud assembly that has been installed in the Palo Verde Unit 1 reactor vessel after it was modified.

Certified material test reports 2)
Weld procedure qualification records 3
Forming procedures 4)
In-process and final heat treatment procedures 5)
In-process and final cleaning procedures 6)
Fabrication drawings Please inform us within two weeks of receipt of this letter as to when you plan to respond to this request.
