Telephone Conversation between P. Narbut and T. Bradish on May 11, 1984
Dear Sir:
The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation.
At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an Interim Report is attached.
It is now expected that this information will be finalized by August 24, 1984, at which time a complete report will be submitted.

INTERIM REPORT DER 84-30 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNITS 1, 2, 6 3 I. Potential Problem There are two (2) ASME Code Stamped "N" items (ASME Code Class
- 3) on each of the "A" and "B" trains.
They are:

2MDGAE04 Lube Oil Cooler MRG S/N 9-20016-02-3 2.
2MDQAE05 Jacket Water Cooler MFG S/N 9-20016-01&
3.
2MDGBE04 Lube Oil Cooler MFG S/N 9-20016-02-2 4.
2MDGBE05 Jacket Water Cooler MFG S/N 9-20016&2-2 Parts that are non-conforming on each item listed above are:
- 1. Four (4) 1" dia. half (1/2) couplings (these are for three (3) thermowells and an isolation valve).
- 2. One'reducing bushing.

One isolation valve.
(V341, V342, V441, V442) 4.
Two (2) 3/4" dia. half (1/2) couplings for vent and drain lines).
All of=the above parts are within the ASME Code boundaries established by P&ID 13-M-DGP-001, Rev.
10 and the vendor'Cooper Energy Services).
The N-1 Manufacturer 's Code Data Report (American Standard Heat Transfer Division) for each of the items (lube oil and
)acket water cooler) do not list these parts, nor is there any objective evidence supporting documentation, i. e., material certifications, weld documentation, etc. available in the files.
The isolation valve does not have a Code nameplate nor any identification marks.
Heat numbers are not visible, on the half couplings, and there are no weld identification numbers, nor the welders'dentification number, for the attachment welds.
Weld documentation for these welds are also not available.
II. Approach To and Status of Proposed Resolution Bechtel engineering is currently corresponding with Cooper Energy on this problem to determine reportability and technical justification for corrective action.
