The specific information required is identified in'the enclosure.
We ask that you provide responses to the enclosed request.
Within two weeks of receipt of this letter, we request that you advise us as to when the required information will be submitted.

Enclosure Request for Additional Information Palo Verde Nuclear Generating Station Docket.Nos. 50-528, 50-529 and 50-530 We note numerous apparent errors or. inconsistencies between job titles used in FSAR Section 13.1.2 and Figure 13.1-6.
We also note other areas where superseded titles remain, e.g.,
"Vice President of Nuclear Operations" in Sections 13.4', 14.2.2.3.1, and 13.1.1.2.2; and "Manager of Nuclear Operations" in sections 14.2.2.11, 14.2.2.12.1.B.3, 13.1.1.2.2, 13.1.3.1 and 17.2.1.1.3.
Please provide replacement FSAR pages which are appropriately "marked-up" to correct those errors and clarify the actual organizational structure and interrelationships.

This section states the Operations Manager is responsible for operation and maintenance of plant systems.
In view of the latest change in organizational structure, the inclusion of a maintenance responsibility appears to be an error.
Please confirm this, or correct our understanding of the organization.
Please supply Statements of Qualifications/Resume's for the current incumbents of the following positions:
Executive Vice President - ANPP, Vice President - Nuclear, Assistant Vice President - Nuclear, Nuclear Engineering
- Manager, Startup Manager, Transition Manager, Nuclear Operations Licensing Supervisor, Director of Nuclear Operations,'echnical Support Manager, Operations
- Manager, Maintenance
- Manager, Administrative Support Manager, Security Manager, Training Manager, Fire Protection Manager, Unit Operations Superintendents, Compliance Supervisor, STA/ISEG Supervisor and NSG Supervisor.
Please supply Figures 14.2-1 and 14.2-2 (Startup Organization) which are referenced in the text but not provided in the FSAR.
Please supply the correct reference in paragraph 14.2.2.12.1.A.3.
The indicated paragraphs do not exist.
Please define the factors to be considered and acceptance criteria to be used in performiAg the case-by-case qualification of otherwise unqualified startup personnel as described in section 14.2.2.12.1.B.3.
Please supply information concerning the duties, responsibilities and qualifications of the Operations Support Superintendent shown in Figure 13.1-6 reporting to the Operations Manager.
Please provide the information requested in question 7 for the Day Shift.

Please identify the organization staffed with qualified inspectors of safety-related maintenance, and show the relationship of this organization to those of the Director of Nuclear Operations, the PVNGS Opex'ations QA/QC Manager and the Corporate Quality Assurance Manager.

Please furnish copies of corporate safety policies 726B and 726C, revised to reflect the current organizational structure.
We interpret ANSI/ANS 3.1-1978, paragraph 4.2.1 to mean that principal alternates shall have the same training and experience required of the plant manager, including the "experience and equivalent training normally required to be eligible for a Senior Reactor Operator's license whether or not an examination is taken."
Please confirm your agreement with this interpretation as applied to FSAR paragraph 13.1.3.1 and the note "principal alternate" following the position titles Operations Manager and Technical Support Manager.

Please confirm the Unit Operations Superintendents will meet the qualifications of ANSI/ANS 3.1-1978, paragraph 4.2.2, Operations Manager.
i Please update the table of qualifications given in FSAR Section 13.1.3.1 using current position titles.
Also, please add the qualification requirements (per ANSI/ANS 3.1-1978) for the Day Shift Supervisor.

In the response to Question 9 given in your letter of September 30, 1981 (Van Brunt, APS to Tedesco, NRR), you state, "Training in mitigating core damage will be given to STA's and operating personnel from the Manager of Nuclear Operations through the operations chain to and including the licensed operators."
Please confirm this commitment now applies to the equivalent position in the revised organization, i.e., the Director of Nuclear Operations.

FSAR Section 13.4.1 states, "A detailed description of the PRB is provided in the PVNGS Technical Specification...."
A similar statement is made in Section 13.4.2 regarding the NSG.
Section 16.6 of the FSAR (Technical Specifications Administrative Controls),
- however, does not provide any description of the PRB, and refers to the SAC instead of the NSG.
- Further, the description of the SAC given in FSAR Section 16.6.5.2 does not agree with the description of the NSG given in FSAR Section 13.4.2.
Please clarify these matters for the record.

FSAR Section 13.4 does not address the review function of the STA/ISEG.
This function should be described in this section of the FSAR or reference should be made to an applicable description of your commitments in this area.
Please revise Section 13.4 to address this concern.
17.
Please confirm the qualifications of members of the Nuclear Safety Group and of the Group Supervisor will conform to the requirements set forth in paragraphs 4.7.1 and 4.7.2 of ANSI/ANS 3.1-1978.
