Evaluation of Potential Missile Hazards From Fan Blade Assembly Failure - Palo Verde, Units 1, 2(arid 3 On September 24, 1982, your office informed the NRC of a failure of a blade on a'Joy Fan within the Palo Verde Unit 1 containment building.

An'nterim report (DER 82-53) regarding this failure was Submitted by letter, dated October 25, 1982.

By letter, dated December 22, 1982, you informed the NRC that a final report would be submitted by February 25, 1983.

In order for our office to complete an evaluation of the potential missile hazards that could result from the failure of the fan blade, we request that the following information be provided by February 25, 1983.

1.

A detailed description of the cause of the fan blade failure and the resulting missile.

2.

An evaluation of whether;.'additional physical protection fe'atures, or other measures, are required for protection of safety-related equipment against internal missiles resulting from fan failures.

3.

The extent to which the FSAR (Sections 3.5.1. 1 and 3.5.1.2) may need to be revised as'.; a result of the above evaluation.

If any of the above requested information is to be provided in the final report, then the response to this letter can refer to the final report as pro'v'iding that portion of the answer.
