SUBJECT:
SUtSARY OF SITE AUDIT OH PUh1P 8 VALVE OPERABIL'ITY A site audft on pump and valve operability assurance was conducted at the Palo Verde site on August 3-5, 1982.
The purpose of the audit was to confirm the acceptability of the operability assurance program and fts implementation for safety-related pumps and valves.
An exit meeting on the staff's findings, was held on August 6, 1982.
The attendees for the site audit and for the exit meeting are listed fn Enclosures 1
and 2, respectively.
The meetings are summarized as follows.
Pump 8 Valve Operabflfty Audit (August 3-5)
On the first day of the audit, the staff identified the pumps and valves that were to be audited.
Of the 12 items previously listed in our letter of July 6, 1982, all but two valves (Item Hos.
6 and 7 fn Enclosure 1 to the July 6, -1982 letter) were audited.
For each item audited, the staff reviewed system and manufacturer's
- drawings, design and purchase specifications, qualification documentation and test reports.
The staff also physically inspected each item as installed in the plant.
The applicant briefly discussed its program for keeping track of plant surveillance and maintenance activities.
For this purpose, the applicant has developed a compute program which fdentffies the frequency and the next scheduled date for each activity and the date when ft was last performed.
The program will also be used to alert management.ff a scheduled surveillance check or maintenance operation eras not perfortted.
Exit hfeetfng (August 6)
He informed the applicant that the staff had not found any basis during the course of the audit that would require a follow up audit of pump and valve operability assurance for Palo Verde.
He did identify the fol'lowing four open items requiring applicant action which the applicant agreed to provide.

Charging Pump (NSSS Scope) - Provide justification describing why the plunger cover on the pump does not conform to the gas tight requirement given in the general specifications.
(2)
Pneumatic Globe Valve (NSSS Scope) - Since the manual operator for the valve can be positioned such that it can override the automatic function of the actuator, provide justification describing how the valve will perform its safety function when required to do so.
(3)
Main Steam Isolation Valve (BOP Scope) - Correct serial and model number inconsistencies between the SgRT master list, PVORT long form and the valve actuator tag.
(4)
Hydrogen Purge Valve (BOP Scope) - Since it appears that the automatic function may not override a manual position if the actuator is engaged in a locked position, provide justification describing how the valve will perform its safety function when required to do so.
