ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT /33 I

Descri tion of Deficienc Forty (40) No.

11 dowels were inadvertently omitted from Auxiliary Building Wall Elevation 70'-0" prior to concrete placement.

The nonconforming condition was discovered by a Bechtel Field Engineer and documented on NCR No. C-A-1957.

This condition was caused by the personnel placing the rebar and the inspectors not following the design drawings.

II Anal sis of Safet Em 1'ications This condition is considered a reportable deficiency based on two considerations:

l. If not detected and corrected as indicated, the wall could possibly experience local structure failure under seismic conditions.

2.

This condition was not detected by the established inspection program.

III Corrective Action The condition has been corrected by adding dowels, prior to concrete placement, which tie into the slab.

This repair provides a design equivalent to the original design.

Experience to date indicates that this is an isolated case.

Further investigations into this type of defect is not con-sidered necessary.

All.personnel associated with the placement of rebar have been instructed to follow the drawings carefully and pay close attention to details.
