CPPR-141, 142 Notice of'iolation Based on the results of the NRC inspection conducted on July 31-August 3, 19?9, it appears that one of your activities was not conducted in full compliance with NRC requirements as indicated below:

10 CFR 50, Appendix B, Criterion V. Instructions, Procedures and

Drawings, as implemented by PSAR Section 17.1A.5, requires that activities affecting quality be accomplished in accordance with instructions, procedures or drawings appropriate to the circumstances.

Bechtel Specification 13-Pll-209.

"Design Specifications for Nuclear Pipe Supports,"

Paragraph 4.5.5.1 states "Fillet welds shall be of the specified size irith full throat and legs of uniform size."

Bechtel Specification 13-PN-201, "Technical Requirements for Shop Fabrication of Nuclear Service Piping," Paragraph 4.6.4.1 states the same requirement as above.

Bechtel Work Plan Procedure/Quality Control Instruction (WPP/QCI)

No. 201.1, Paragraph

7. 2 states in part, that "Pipe supports shall be installed per the design drawing...and this procedure."

Para-graph 14.3 states that "The welding QCE shall perform his associated inspection and documentation in accordance with the WPP/QCI No.

101.0;" Appendix I, Paragraph C.l states in part, that "Welding QCE shall review, stamp and date the CIP for Task No. 8.0.

This shall signify that the welding is acceptable and documentation is complete II Contrary to the above requirements, on June 14, 1979 the NRC inspector identified an undersize fillet weld on pipe support No.

ISI-079-K-005.

This weld was a shop weld.

On August 2, 1979, the NRC inspector identified an undersize fillet (shop) weld on pipe support No. ISI-129-H-003, an incomplete fillet (shop) weld. on pipe support No. IEC-069-H-008, and an incomplete and missing (field) weld on pipe support No.

1EW-023-H-033.

- Construction inspection planning reports for all of the above supports were complete indicating acceptance.

This is an infraction.
