This letter refers to the Inspection conducted by Mr. L. Vorderbrueggen and members of your staff on July 31-August 3,
- 1979, as documented in your letter of September 19, 1979 of activities authorized by Nuclear Regulatory Commission (NRC) Construction Permits Nos.
CPPR-141, 142 and 143.
During this Inspection, one item of noncompliance was identified.
Our response to this item of noncompliance (Infraction) is presented in the enclosed Attachment A.

- 142, 143 Notice of Violation Based on the results of the NRC Inspection conducted on July 31-August 3, 1979, it appears that one of your activities was not conducted in full compliance with NRC requirements as indicated below:
- 10CFR50, Appendix B, Criterion V, Instructions, Procedures and
- Drawings, as implemented by PSAR Section 17.1A.5, requires that activities affecting quality be accomplished in accordance with instructions, procedures or drawings appropriate to the circum-stances.
Bechtel Specification 13-PM-209, "Design Specifications for Nuclear Pipe Supports",
Paragraph 4.5.5.1, states "Fillet welds shall be of the specified size with full throat and legs of uniform size."
Bechtel Specification 13-PM-201, "Technical Requirements for Shop Fabrication of Nuclear Service Piping", Paragraph 4.6.4.1, states the same requirement as above.
Bechtel Work Plan Procedure/Quality Control Instruction (WPP/
QCI) No. 201.1, Paragraph 7.2, states, in part, that "Pipe supports shall be installed per the design drawing....and this procedure.";
Paragraph 14.3 states that "The welding QCE shall perform his associated inspection and documentation in accor-dance with the WPP/QCl No. 101.0"; Appendix I, Paragraph C.l,
- states, in part, that "Welding QCE shall review, stamp and date the CIP for Task No. 8.0.
This shall signify that the welding is acceptable and documentation is complete...."
Contrary to the above requirements, on June 14, 1979, the NRC Inspector identified an undersize fillet weld on Pipe Support No. ISI-079-H-005.
On August 2, 1979, the NRC Inspector identi-fied an undersize fillet ('shop) weld on Pipe Support No. ISI-129-H-003, an incomplete fillet ('shop) weld on Pipe Support No. IEC-069-H-008, and an incomplete and missing '('field) weld on Pipe Support No. IEW-023-H-033.
Construction inspection planning reports for all of the above supports were complete indicating acceptance.
This is an Infraction.

Corrective Action Taken Bechtel Quality Control and Field Engineering personnel completed a
100% reinspection on August 15, 1979 of installed pipe supports to verify that the fillet welding is acceptable.
The inspection find-ings, including the three discrepancies identified by NRC Inspectors have been documented on, Nonconformances P-A-566, 567 and 568.

Ste s Taken to Avoid Further Violations In the review of these findings and other findings documented by an Arizona Public Service Company audit of June 28, 1979, it was deter-mined that these discrepancies were caused by (1) Bechtel Quality Control personnel being remiss in accepting incomplete and under-sized welds, (2) a Bechtel weld symbol drawing that was not easily understood, (3) pipe hanger suppliers/vendors not performing 100%
inspections on completed hangers, and (4) the complete reliance of Bechtel field personnel on the supplier's inspection and Bechtel Supplier Quality Representative's activities for assurance that pipe hangers meet the specification and drawing requirements.
As a result of this review, the following corrective actions were taken:
a.
Bechtel Construction Work Plans Procedures/Quality Control
'-Inspection (MPP/QCI) No. 201.1, "Nuclear Pipe Hangers and Supports Installation"', Revision 5, has been revised as follows:
(1)
Section 8.0, Paragraph 8.1.1, added "Drawing No.
13-,S-Z-AS-.519 shall be used to interpret weld symbols for pipe support assemblies and pipe racks."
This was added to the procedure to reference the weld symbol drawing so that in-stallation drawings can be fully understood.
(2)
Sect'ion 14.0 revised to read:
14.3 "...of pipe support welding, Inspection shall include visual examination of Vendor welds for size and length."
(3)
Exhibit 201.1-"7 (Revision 8) added inspection verification of vendor weld size and length.

b.
All Quality Class Q supports installed to date have been inspected and all welds not meeting minimum requirements are to be reworked to comply with the minimum requirements of drawings and codes (ASME/AWS), or dispositioned case-by-case by means of existing NCR procedures.
Inspection in-cludes use of weld fillet gages.
c.
Suppliers have been advised to be sure that their quality control personnel are performing a 100% visual inspection of fillet welds and a random measurement of welds using weld fillet gages.
d.
Bechtel Supplier Quality Representatives (SQR's) have increased their surveillance of supplier's activity to verify fillet weld compliance.
This includes the use of weld fillet gages by the SQR.
e.
A training session was conducted on August 17 and 20, 1979 with affected Welding QCE's on the inspection of nuclear pipe supports.
The training consisted of the method of inspection, interpretation of weld symbols, drawing require-ments and proper use of weld fillet gages.
Emphasis was placed on the strict adherence to:the design requirements.
Documentation of the training session has been placed in the individual's respective training file.
3.
Date of Pull Com liance Item a., Procedure and Drawing Changes, were completed and are being fully implemented as of October 19, 1979.
Item b., is an on-going activity.
Items c. and d., all of the pipe hanger suppliers have been notified to be certain they are performing 100% inspections.
These activi-ties should be fully implemented November 15, 1979 and will be veri-fied by Quality Assurance follow-up audits.
Item e., the training was completed August 20, 1979.
