Box 21666 Phoenix, Arizona 85036 Docket No. 50-529 Construction Permit No. CPPR-155 Qe Notice of Violation Based on the results of an NRC inspection conducted November 29-December 2
- and December 5-7, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Construction Per-mit No. CPPR-155 as indicated below:
Appendix B of 10 CFR 50, Criterion IX, requires that special processes be "...accomplished by qualified personnel using qualified procedures...."
The PVNGS PSAR (Admendment 17, PP 17.1A-36) implements this criter ion and requires that:
"Special processess shall be performed...by qualified personnel in accordance with...procedures...."
Bechtel lJPP/gCI No. 56.0, "Cadweld Splicing of Reinforcing Steel, " Paragraph 5..5, states that, "Prior to assembling the Cadweld sleeve into position on rebar ends, the bar ends shall be marked...."
Contrary to the above requirements, on November 30, 1978, a
Cadweld sleeve was placed and pinned without prior marking of the rebar.
This horizontal Cadweld was being made on the West side of the Unit 2 primary shield wall.
Cadweld splicing per-sonnel stated that bar marking after sleeve placement has been used as an optional procedure for horizontal.rebars.
This item is an infraction.
