ENCLOSURE 1 NOTICE OF VIOLATION Washington Public Power Supply System Washington Nuclear Project-2 Docket No.:

NPF-21 During an NRC inspection conducted from December 27, 1998, through February 6, 1999, one violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, Revision 1, the violation is listed below:
Technical Specification 5.4.1.a. requires, in part, that written procedures shall be established, implemented, and maintained covering applicable procedures recommended in Regulatory Guide 1.33, Revision 2, Appendix A, February 1978.
Regulatory Guide 1.33, Item 1.d, requires that procedures are established covering procedure adherence.
Procedure SWP-PRO-01, "Description and Use of Procedures and Instructions,"
Revision 2, Section 3.6.2 b, stated, in part, "STRICT ADHERENCE to approved written procedures is the Supply System standard for operating and maintaining the plant safely
... Departure from other procedural direction is accomplished only after concurrence from supervisor/management and procedure change(s)...."
Procedure 10.2.53, "Seismic Requirements for Scaffolding, Ladders, Man-Lifts, Tool Gang Boxes, Hoists, and Metal Storage Cabinets," Revision 16, paragraph 7.1.3.d states to, "Obtain Engineering approvals if scaffolding needs to be deviated from this PPM [procedure]."
Contrary to the above, on December 30, 1998, scaffolding on standby gas treatment system Train B was found installed in a configuration which deviated from Procedure 10.2.53 without the required engineering evaluation.

2 Procedure 10.5.9, "Hydraulic Control Unit Refurbishment," Revision 0, Temporary Change Notice 99-12, step 6.16.3, stated, in part, "Adjust the switch actuator to the point where it just loses contact with the switch. Record installation on Attachment 8.1."
Contrary to the above, on January 24, 1999, electricians failed to implement Procedure 10.5.9, in that step 6.16.3 was recorded as being completed without properly adjusting the limitswitch.
This is a Severity Level IVviolation (Supplement I) (50-397/98025-01).
The NRC has concluded that information regarding the reason for violation, the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance will'be achieved is already adequately addressed on the docket in the inspection report. your position.
In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector at the facilitythat is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation

If you contest this enforcement action, you should also provide a copy of your response, with the basis for your denial, to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
