SUBJECT: NRC INSPECTION REPORT 50-397/98-25 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
This refers to the inspection conducted from December 27;1998, through February 6, 1999, at
your Washington Nuclear Project-2 facility. The enclosed report presents the results of this
inspection.
Based on the results of this inspection, the NRC has determined that a violation of NRC
requirements occurred that included two examples.
This violation is cited in the enclosed
Notice of Violation (Notice), and the circumstances surrounding the examples are described in
~
the subject inspection report. These examples are of concern because the lack of attention to
procedure requirements resulted in hydraulic control unit indication not functioning as required
following maintenance and scaffolding being supported by a non-load bearing member of a
Class
1 component without an engineering evaluation.
The NRC has concluded that information regarding the reason for the violation and the
corrective actions taken and planned to correct the violation and prevent recurrence is already
adequately addressed
on the docket in the enclosed inspection report. Therefore; you are not
required to respond to this letter unless the description therein does not accurately reflect your
corrective actions or your position.
In that case, or ifyou choose to provide additional
information, you should followthe instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, its
enclosures, and your response willbe placed in the NRC Public Document Room (PDR).

with you.
