Qa ENCLOSURE 1 NOTICE OF VIOLATION Washington Public Power Supply System Washington Nuclear Project-2 Docket No.:

50-397 License No.:

NPF-21 During ari NRC inspection conducted from November 22 through December 26, 1998, two violations of NRC requirements were identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, Revision 1, the violations are listed below:.

A.

10 CFR Part 50, Appendix B, Criterion III, "Design Control," states:

"Measures shall be established to assure that the design basis... as specified in the license application...

are correctly translated into drawings."

WNP-2 Final Safety Analysis Report, Section 7.3.1.1.1.3, "Low Pressure Core Spray,"

which was included in the license application states:

"Operator information displays are shown in Figure 7.3-9." Figure 7.3-9 indicates that the low pressure core spray out-of-service annunciator is supplied by: (1) Battery B1-1 out of service, (2) Diesel Generator 1 out of service, and (3) Service Water A out of service, amongst other inputs.

r Contrary to the above, as of December 7, 1998, the design basis specified in the license application was not correctly translated into drawings.

Specifically, Drawing EWD-8E-011, "Electrical Wiring Diagram Low Pressure Core Spray System Bypass 8 Inoperable Status Indication Display ANN-ANN-LPCS,"Revision 16, did not indicate that Battery B1-1, Diesel Generator 1, and Service Water A out-of-service signals supply the LPCS out-of-service annunciator.

As a result, the configuration of the LPCS out-of-service annunciator was not in agreement with Final Safety Analysis Figure 7.3-9.

This is a Severity Level IVviolation, (Supplement I) (50-397/98023-01).

B.

License Condition 2.E states: "The licensee shall fullyimplement and maintain in effect all provisions of the Commission-approved physical security, guard training and qualification, and safeguards contingency plans...."

Commission-approved, Physical Security Plan, Revision 37, dated January 1, 1998, indicates that the Physical Security Plan is fullyimplemented by details in S'ecurity Procedures and Contingency Plans.

Procedure SPIP-SEC-02, "Central and Secondary Alarm Stations," Revision 0, Section 3.2, "Secondary Alarm Station (SAS)," paragraph j. states:

"Security manuals permanently posted at SAS are the only reading materials authorized for this post."

Contrary to the above, on December 14, 1998, the on-duty SAS security officer was engaged in reading unauthorized material, a magazine.

This is a Severity Level IV violation (Supplement I) (50-397/98023-03).

II Washington Public Power Supply System The NRC has concluded that information regarding the reasons for the violations, the corrective actions taken and planned to correct the violations and prevent recurrence, and the date when full compliance willbe achieved is already adequately addressed on the docket in the enclosed inspection report.

However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions o your position.

In that case, or if you choose to respond,'clearly mark your response as a "Reply to a Notice of Violation," and send it the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator,,Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident Inspector at the facilitythat is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

If you contest this enforcement action, you should also provide a copy of your response, with the basis for your denial, to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.

i If you choose to respond, your response willbe placed in the NRC Public Document Room (PDR). Therefore, to the extent possible, the response should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.

If personal privacy or proprietary information is necessary to provide an acceptable

response, then please provide a bracketed copy of your response that identifies the information that should be protected and a redacted copy of your response that deletes such information.
