SUBJECT: NRC INSPECTION REPORT 50-397/98-23 AND NOTICE OF VIOLATION
Dear Mr. Parrish:
This refers to the inspection conducted from November 22 to December 26, 1998, at your
Washington Nuclear Project-2 reactor facility. The enclosed report presents the results of this
inspection.
Based on the results of this inspection, the NRC has determined that two violations of NRC
requirements occurred.
These violations are cited in the enclosed Notice of Violation (Notice)
and the circumstances surrounding them are described in detail in the subject inspection report.
The first violation involved a failure to establish adequate bypassed and inoperable status
indication for the low pressure core spray system.
This violation was of concern because it was
not identified during your Final Safety Analysis Report upgrade program. The second violation
involved a security officer reading unauthorized material, which could detract from his shift
responsibilities.
The NRC has concluded that information regarding the reason for the violations and the
corrective actions taken and planned to correct the violation and prevent recurrence is already
adequately addressed
on the docket in the enclosed inspection report. Therefore, you are not
required to respond to this letter unless the description therein does not accurately reflect your
corrective actions or your position.
In that case, or if you choose to provide additional
information, you should followthe instructions specified in the enclosed Notice.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room.

with you.
