SUBJECT: NRC INSPECTION REPORT 50-39?/98-03 AND NOTICE OF VIOLATION.

Dear Mr. Parrish:

From February

1 to March 14, 1998, a routine resident inspection was conducted at your

Washington Nuclear Project-2 reactor facility. The enclosed report presents the scope and

results of that inspection.

During the inspection period, the conduct of licensed activities at your WNP-2 facilitywas

characterized by generally safety-conscious operations and maintenance, with appropriate

support from plant engineering.

However, four violations and one noncited violation of NRC

requirements were identified, indicating performance lapses in operations, engineering, and fire

protection. The first two violations involved an organizational knowledge deficiency on the use

and applicability of the grace period allowed for Technical Specification surveillance testing.

The third violation involved your program to monitor primary coolant sources outside

containment.

The fourth violation involved inadequate control of transient combustibles and

indicates that actions taken in response to a recent similar violation were ineffective.

These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detail in the enclosed report. The NRC has

concluded that information regarding the*reasons for Violations A and B, the corrective actions

taken and planned to correct the violations and prevent recurrence are already adequately

addressed

on the docket in the enclosed report. Therefore, you are not required to respond to

this letter concerning those two violations unless the description therein does not accurately

reflect your corrective actions or your position.

In that case,'or ifyou choose to provide

additional information, you should followthe instructions specified in the enclosed Notice.

Please note that you are required to respond to this letter and should followthe instructions

specified in the enclosed Notice when preparing your response to Violations C and D. The

NRC will use your response,

in part, to determine whether further enforcement action is

necessary

to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.?90 of the NRC's "Rules of Practice," a copy of this letter, its

enclosures,

and your response willbe placed in the NRC Public Document Room.
