Dear Mr. Parrish:

On June 3-7.
1996. the
NRC completed
an inspection at your Washington Nuclear
Project-2 reactor facility.
The enclosed
report presents
the scope
and
results of that inspection.
This inspection
focused
on the installation and testing of digital retrofits,
including the first of a kind installation of adjustable
speed drives for the
pumps
and
a digital feedwater control system.
Although the installation of the adjustable
speed drive modification was well
controlled, the inspection
revealed
a number of vulnerabi lities associated
,with the design
and installation of the digital feedwater retrofit.
The
vulnerabilities included minimal licensee
involvement in the design of the
system,
planned reliance
on the vendor for day-to-day maintenance
support for
the system,
and fragmented test programs.
The apparent
lack of consideration
of analog sensor
noise in the designs of both digital retrofits was
particularly disappointing in light of industry experience
in this area.
In accordance with 10
CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the
NRC Public Document
Room
(PDR).
