WATER SYSTEM OPERATIONAL PERFORMANCE

INSPECTION

(50-397/93-201)

We are forwarding the report of the service water system operational

performance

inspection

conducted

by the U.S. Nuclear Regulatory

Commission

(NRC) staff from February

1 through ll, 1993.

The activities inspected

were

authorized

by

NRC Operating

License

NPF-21 for Washington Nuclear Plant, Unit

2 (WNP-2).

At the conclusion of the inspection,

an exit meeting

was held

where the team's findings. were discussed

with you and

members of your staff.

The inspection

team assessed

the operational

performance of the service water

system

(SWS), including the design,

operations,

maintenance,

testing,

and

surveillance of the system

and evaluated

implementation of the quality

assurance

and corrective action programs.

The team also

assessed

the planned

or completed

actions in response

to

NRC Generic Letter 89-13,

"Service Water

System

Problems Affecting Safety-Related

Equipment,"

issued

on July 18,

1989.

The team found the design of the service water system to contain

adequate

margin to account for some calculational

uncertainties

and system degradation.

However, the team identified several

deficiencies

in your staff's performance

relative to the service water system.

Specifically, the team identified that

some

known deficiencies

were either not being properly documented,

or when

documented,

not fully resolved.

Host significant of these

was

a hammering

problem associated

with the service water loop isolation val'ves

SW-V-12A/B.

Although this problem was initially documented

in May of 1991, the cause

and

extent of the problem

and the possible ramifications were not understood

by

your personnel

at the time of this inspection.

Your failure to document

and

resolve

known deficiencies is an issue that has

been identified previously by

both

NRC and

WNP-2 auditors

and

one that warrants your continued

management

attention.

The team also identified

a lack of procedures

to cope with spray

pond icing,

a

biofouling program that had not been proceduralized

or adequately controlled,,

and

some degradation

in the material condition of the service water system.
