conducted
by Messrs.
R.
C. Sorensen,
D. L,
Proulx,
and
R.
C. Barr of this office on September
18 - October 22,
1991 of
activities authorized
by
NRC Operating
License
No. NPF-21,
and to the
discussions
of findings held by the inspectors with Mr. J.
Baker
and other
members of the Supply System staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted
of selective
examinations
of procedures
and representative
records,'interviews
with
personnel,
and observations
by the
inspectors.'ased
on the results of this inspection,
one of your activities appeared
to be
in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
This violation involved not following the plant startup
'rocedure
by reopening
main steam line drains before returning to cold
shutdown.
This violation indicated
an apparent'weakness
in the oversight
functions of the Supply System in that, despite
the extensive
management,
Ouality Assurance,
and Operations staff presence
in the control
room during
the occurrence,
the violation was noted only by the
NRC inspector.
In
addition,
management
did not adequately
communicate its concerns
to minimize
(or track) the number of thermal
cycles
imposed
on the main steam line drains.
You are required to respond to this letter and should follow the instructions
ln the enclosed
Notice when preparing your response.
In your response,
you
should document
the specific actions
taken
and
any additional actions
you
plan
to prevent recurrence.
After reviewing your response
to this Notice,
including your proposed corrective actions
and the results of future inspec-
tions, the
NRC will determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
this letter, its enclosures,
and your response will be placed in the
NRC
Public Document
Room.
