APPENDIX A NOTICE OF VIOLATION Washington Public Power Supply System (WPPSS)

NPF-21 During an NRC inspection conducted on August 8-10,
- 1990, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C
(1990), the violation is listed below:
10 CFR Part 20. 103(a)(3) requires, in part, that for the purpose of determining compliance with the requirements of this section the licensee shall use suitable measurements of concentrations of radioactive materials in air for detectinq and evaluating airborne radioactivity in restricted areas and in addition, as appropr)ate shall use measurements of radioactivity in the body...
as may be necessary for timely detection and assessment of individual intakes of radioactivity by exposed individuals.
Furthermore, this part requires that when assessment of a particular individual s intake of radioactive material is necessary,
'ntakes less than those which would result from inhalation for 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> in any one day or for 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> in any one week at uniform concentrations specified in Appendix B, Table I, Column 1 need not be included in such assessment, provided that for any assessment in excess of these amounts the entire amount is included.
Contrary to the above, on April 30 and May 20, 1990, individuals were exposed to radioactive materials such that significant quantities were measured in their lungs and an accurate assessment of their intake was not made until August 2, 1990.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C.
20555 with a copy to the Regional Administrator, Region V, and if applicable, a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results
- achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the 9009140149'00824 PDR ADOCK 05000397 Q
license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.
Where good cause is shown, consideration will be given to extending the response time.
