APPENDIX A NOTICE OF DEVIATION Washington Public Power Supply System Post Office Box 968

Richland, WA 99352 Docket No. 50-397 License No.

NPF-21 During an NRC inspection conducted on October 2 - November 12, 1989 a

deviation from a commitment to the NRC was identified.

In accordance with the "General'tatement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1988), the deviation is listed below:

The WNP-2 Final Safety Analysis Report, Amendment No. 36, dated

December, 1985, Table 9.5-8, Locations of Emer enc Li htin

, shows normal AC lighting being provide to Resi ua eat Remova RHR) Valve Room A'1, located on elevation 471'f the reactor building.

Contrary to the above, all lights in this room were burned out on October 31, 1989; thus no normal AC lighting was provided to it.

This is a Deviation.

The Washington Public Power Supply System is hereby requested to submit a

written statement or explanation to the U.S. Nuclear Regulatory Comission, ATTN:

Document Control Desk, Washington DC, 20555, with a copy to the Regional Administrator, Region V, and a copy to -the NRC Senior Resident Inspector, WNP-2, within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a "Reply to a Notice of Deviation" and should include the following: (1) the reason for the deviation, if admitted, (2) the corrective steps which have been taken and the results

achieved, (3) the corrective steps which will be taken to avoid further deviations, and (4) the date when your corrective actions will be completed.
