APPENDIN A NOTICE OF VIOLATION Washington Public Power Supply System P.O.

NPF-21 During an NRC inspection conducted October 2-5, 1989, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C
(1989), the violation is listed below:

10'FR 50.54(q) requires that licensees follow and maintain in effect emerqency plans which meet the standards in 50.47(b) and the requirements in Appendix E to Part 50.
Paragraph IV.F.5 of Appendix E to Part 50 requires that any weak or deficient areas identified during drills or exercises be corrected.
Contrary to the above requirement, as of October 4, 1989, the licensee had not corrected a weak area, involving the adequacy of notifications to plant personnel, identified during the annual emergency exercise conducted on September 13,'988.
This a Severity Level IV violation (Supplement VIII).
Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulato'ry Commission, ATTN: Document Control Desk, Mashington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector, WNP-2, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results
- achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an. order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other actions as may be proper should not be taken.
Consideration may be given to extending the response time for good cause shown.
