Bosted
and
R. C. Sorensen
of this office on July 10 - August 23,
1989
and
a special
inspection
by Messrs. J. L. Crews
and
D. F. Kirsch of this office on July
12
and 13, 1989, of activities authorized
by
NRC Operating License
No. NPF-21,
and to the discussions
of findings held by the inspectors
with Mr. C. Powers
and other members of the Supply System's staff at the conclusion of the
inspections.
Areas
examined during this inspection
are described
in the enclosed
inspec-
tion report.
Within these areas,'the
inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel
and observations
by the inspectors.
Recent events
have raised
concern regarding your staff's ability to exercise
effective control of maintenance activities.
At our management
meeting
on
August 8 to discuss your latest
SALP assessment,
we focused
on identified
weaknesses
in the maintenance
area.
At that time,
we recommended
that
increased
management
attention
be directed to that area.
The reactor
on August
17 and the violation cited in the enclosed
Notice of Violation also
resulted
from failure to follow maintenance
or surveillance
procedures,
further illustrating this concern.
We urge you to take prompt steps to
provide improved performance in this area.
In your reply to the enclosed
Notice of Violation, in addition to the specific responses
directed,
please
discuss
the steps
you have taken or plan to take to ensure
improved control
of maintenance
and surveillance activities.
We wish to recognize
the efforts the Supply System
has
been investing in
engineering
improvements.
In particular,
as discussed
in paragraph
16 of
the enclosed report, the safety
system functional inspection
(SSFI) of the
'low pressure
core spray system which was conducted
by your staff was note-
worthy in its scope
and depth
and in the significance of the findings which
resulted.
Several
fundamental
concerns
were identified by your SSFI, however,
which require senior management
consideration for resolution of policy.
These
areas
are addressed
in more detail in paragraph
16 of the enclosed
report and
include:
(1) the applicability of findings to other plant safety systems
and
the assessment/resolution
of that applicability; (2) the establishment
and
implementation of a Supply System Configuration Control Program;
(3) improve-
ment of plant tracking/management
information systems
to make these

redundant
and more user-friendly, with flexible sorting systems;
4) policy
regarding
use of the Vital Maintenance
Work Request
process;
and
5) policy
regarding field modification of maintenance
procedures.
Please
assess
these
issues
and provide us with the results of your assessments,
as well as
planned corrective actions, within 30 days of the date of this le&er.
Based
on the results of this inspection, it appears
that one of your
activities
was not conducted
in full compliance with NRC requirements,
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.

Your response
to this Notice is to be submitted in accordance
with the
provisions of 10 CFR 2.201
as stated in Appendix A, Notice of Violation.
Enforcement action regarding the issue discussed
in paragraph
17 of the
enclosed report will be the subject of separate
correspondence.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosure,
and
your response will be placed in the
NRC Public Document
Room.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget
as
required
by the Paperwork
Reduction Act of 1980,
PL 96-511.
