SUBJECT: NRC INSPECTION REPORT 50-397/98-09 AND NOTICE OF VIOLATION

Dear Mr. Parrish:

From April26 through June 6, 1998, an inspection was conducted at your Washington Nuclear

Project-2 reactor facility. The'inspection period captured a significant portion of activities

associated with Refueling Outage R13 and, as such, focused upon the conduct and control of

maintenance.

'The enclosed report presents the scope and results of that inspection.

Overall, outage activities.were well coordinated with generally strong safety performance in

plant operations.

However, weak procedure quality and adherence to procedures during

maintenance resulted in three separate violations of NRC requirenients.

The procedure

adherence violations included: 1) failure to verify temperature requirements prior to liftingheavy

loads with the reactor building overhead crane; 2) failure to isolate a reactor protection system

pressure switch during testing of excess flow check valves; and 3) failure to obtain an

independent verification during testing of excess flowcheck valves. A separate violation for

procedure inadequacies was also identified during testing of excess flow check valves and was

related to the establishment of and restoration from test conditions.

A fourth violation was identified when the condition of a partially elevated fuel assembly was

accepted without an adequate review of the plant's design basis.

The review would have

identified the requirement for the full insertion of the assembly in the spent fuel pool storage box

to meet the assumptions of the spent fuel pool criticality analysis. Afifthviolation was identified

when it was determined that you had decreased

the effectiveness of the WNP-2 emergency

plan by reducing the number of on-shift health physics technicians and overburdening the

onshift chemistry technician with health physics responsibilities in an emergency.

The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding these violations are described in detail in the enclosed report.

whether further enforcement action is necessary to ensure compliance with regulatory

requirements.
