conducted
by Messrs.

W. Caldwell

C. A. Clark of this office on August 29 through'eptember
30,
1988, of
activities authorized
by
NRC License

and to the discussion of our
findings held
by the inspectors
with Messrs.
Bouchey and Baker,
and other
members of the Supply System staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
,
examinations
of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appears
that
some of your
activities were not conducted
in full compliance with NRC requirements
as set
forth in the Notice of Violation and Notice of Deviation, enclosed
herewith
as
Appendices
A and
B respectively.
Your response
to these
Notices is to be submitted in accordance
with the
provisions of 10 CFR 2.201
as stated
in Appendix A, Notice of Violation, and
Appendix B, Notice of Deviation.
In addition to the discrepancies
identified in the Notice of Violation, the
potential for the nonconforming installation of your control rod hydraulic
control units
was originally brought to your attention
letter, dated October 20,
1986,
and later by
NRC Information Notice No. 87-56,
dated
November 4,
1987.
During this inspection, it was identified that
a
walkdown, performed in January
1988, of the hydraulic control units found
several
discrepancies
which were not documented
in a nonconformance
report
and
which remained
uncorrected
at the time of the inspection.
These discrepancies
still had not received
an adequate
seismic
and operability evaluation
as of
September
23,
1988,
and remained
uncorrected
and unresolved until September
30,
1988,
when yo'ur Plant Operating
Committee approved
a Justification for
Continued Operation for the last remaining nonconforming condition.
The lack of aggressive
action to resolve the hydraulic control unit concerns
raises
serious
questions
regarding
the implementation of your processes
for
evaluating
vendor information and industry experience
(Operating
Experience
Review (OER) Program), effecting timely corrective action for discrepancies,
and assessing
questions
of safety
system operability.
