Meeting
This refers to the Management
Meeting held with members of the Supply System
staff at the Region
V office on June 7, 1988.
The purpose of this meeting
was
to discuss
items of interest pertaining to activities authorized
by NRC License
Subjects
discussed
during the meeting are
summarized in the enclosed
meeting report 50-397/88-23.
Based
on your discussion
during this meeting,
as substantiated
by the internal
reviews the Supply System
has conducted,
your management
appears
to have
defined appropriate
corrective actions to bring about
needed
improvements
in
the design engineering
area.
Recent
enhancements
in the guality Assurance
area,
including organizational
improvements
discussed
during this meeting,
also
appear to have
been appropriately defined.
We look forward to seeing
the
results of these efforts in making the engineering
and gA organizations
the
effective forces they should
be in plant operations activities.
On the other hand,
as discussed
during our meeting,
we are concerned
about the
number and type of personnel
error events
experienced
at WNP-2 thus far in 1988
(as identified in the enclosed report), especially in view of the maturity of
the plant and the extensive operating
experience
represented
in your plant
staff.
Several
of the errors
by the Operations
Department
are especially
significant because
of their fundamental
nature.
Specific examples
are the
reactor vessel
level control problems experienced
on February
13 and the resin
spill which occur red on May 12.
Subsequent
to our meeting,
on June 13, 1988,
you experienced
an additional
event involving uncertainties
and insufficient
attention to procedures
which resulted in draindown of the drywell spray piping
into the drywell.
You discussed
actions being taken to address
these
personnel
performance
issues,
including reemphasis
of personal
responsibility,
intended
management
changes,
and increased
accountability.
We will be following these
initiatives closely as
we continue to monitor your staff's performance
during
the next
SALP period.
Within the scope of personnel
error events,
we are particularly distressed
about recent errors in the preparation of equipment clearances,
an activity
which is fundamental
to effective work control.
These errors
have resulted in
significant personnel
safety hazards,
with serious injury narrowly averted in

one case.
You are requested
to provide, within 30 days of the date of
this letter,
a response
describing
the actions
you have taken or plan to take
to ensure
an effective level of personnel
safety during the performance of
maintenance activities.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosure,
and
your response will be placed in the
NRC Public Document
Room.
The response
requested
by this letter is not subject to the clearance
procedures
of the Office of Management
and Budget as required
by the Paperwork
Reduction
Act of 1980,
PL 96-511.
