discussions
of findings held by the inspectors
with Mr.
C.
Powers
and other
members of the Supply System's staff at the conclusion of the inspection.
Areas
examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel
and observations
by the inspectors.
Based
on the= results of this inspection, it appears
that two of your
activities were not conducted
in full compliance with NRC requirements,
as set
forth in the Notice of Violation, enclosed
herewith
as Appendix A.
Your response
to this Notice is to be submitted in accordance
with the
provisions of 10
CFR 2.201
as stated in Appendix A, Notice of Violation.
In
accordance
with 10
CFR 2.790(a),
a copy of this letter, the enclosures,
and
your response
wi 11
be placed in the
NRC Public Document
Room.
We recognize that the specific design
problems cited in Violation B of the
enclosed
Notice were identified by your plant staff personnel.
However,
we
are concerned that your management
did not recognize
these
(and other design
problems discussed
in the enclosed
inspection report)
as
symptoms of a possible
breakdown in your design control program,
and that
a thorough root cause
assessment
of the problems
was not initiated.
In view of these
observations
and those identified during our August 1987 Safety System Functional
Inspection
(Inspection
Report
No. 87-19),
we request that in your response
you also
provide detailed findings of the guality Assurance
root cause
assessment
of
the design review inadequacies;
corrective actions,
including your schedule
for completion of the corrective actions;
and your assessment
of the quality
of modifications that have
been previously installed in the plant.
The response
directed
by this letter and the accompanying
Notice are not
subject to the clearance
procedures
of the Office of Management
and Budget as
required
by the Paperwork Reduction Act of 1980,
