NPF-21 During an NRC inspection conducted on January 21, 1988 - March 10,
- 1988, two violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are listed. below:
A.
10 CFR 50, Appendix B, Criterion XVI states in part, "Measures shall be established to assure that conditions adverse to quality, such as...
deficiencies...
and nonconformances are promptly... corrected."
This requirement is implemented by Section
- 16. 2. 1 of.the licensee's Operational guality Assurance Program Description (OgAPD).
10 CFR 50.9(a) requires, in part, that information required to be maintained by licensees be complete and accurate in all respects.
Contrary to the above, on February 3, 1988, one of the foam filters identified in NRC inspection report 87-19 had not been removed as reported by the licensee, and additional filters were in the electrical exciter cubicle for the High Pressure Core Spray Diesel Generator.
This is a Severity Level IV violation (Supplement 1).
B.
10 CFR 50, Appendix B, Criterion III states in part, "The design control measures shall provide for verifying or checking the adequacy of design...."
This requirement is implemented by Section 3.2.5 of the OQAPD.
Contrary to the above, the design control measures did not verify the adequacy of design for design change package (DCP) 86-0229-0B, "ATWS Alternate Rod Insertion", in that this DCP was reviewed,
- approved, and issued to plant staff personnel for implementation in February 1988 with inappropriate
- designs, including improper air header vent and purge valve operation, incorrect actuation signal reset logic, and improper separation of Division 1 and Division 2 electrical components.
This is a Severity Level IV Violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit a written statement or explanation to the U.
S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspection office at Washington Nuclear Project 2, within 30 days of the date of this Notice.
This reply should be clearly marked as a
"Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved; (3) corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
Consideration may be given to extending your response time for good cause shown.
