Dear Mr. Spencer:

This is in response to your letter of ttovember 22, 1978, reference (1),

which documented the results of the NRC inspection conducted on October 24-27, 1978 of activities authorized by NRC Construction Permit No. CPPR-93.

This letter identified one (1) item of noncompliance which was categorized as an infraction in accordance with the provisions of reference (2) and required that WPPSS provide a response to this item.

This specific NRC "Notice of Violation", as stated in your letter, and the HPPSS response is provided in Appendix A to this letter.

APPENDIX A Washington Public Power Supply System P.O.

Richland, WA 99352 Docket No. 50-397 Construction Permit No.

CPPR-93 Notice of Violation Based on the results of NRC inspections conducted on October 24-27,

1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC facility License No.

CPPR-93 as indicated below.

This item is an infraction.

10CFR50, Appendix B, Criterion V states, in part, that "Activities affecting quality...shall be accomplished in accordance with these instructions, procedures or drawings..."

Paragraph D.2.5 of the equality Assurance Program documented in the PSAR states, in part, that, "...all project contractors for the nuclear related portions of the plant will be required to have a equality Assurance Program...The program shall include the following items as...applicable to the...construction for which the contractor is responsible".

Paragraph D.2.5.5 which follows states, in part, that, "Activities affecting quality...

shall be accomplished in accordance with...drawings..."

Contrary to the above, on October 25, 1978, lock nuts were not installed on three anchor bolts of a safety related vertical steel structure column at the 606 foot elevation of the Reactor Building.

Installation of lock nuts on the anchor bolts, an activity affecting quality of the structural

column, was specified by the applicable Burns and Roe Drawing Nos.
S727, Revision 13, and S560, Revision 37.

This construction activity was considered to be complete by the project contractor installing the column and its bolting.

Action to 'Correct Deficiency Nonconformance Report (NCR) Number 03896 initiated November 10, 1978 documents this nonconformance.

Lock nuts have been ordered per FAO-13 with an estimated date of delivery of December 29, 1978.

Estimate installation of lock nuts by January 9, 1979.

APPENDIX A (Con't.)

Action to Prevent Recurrence A letter has been issued to all WNP-2 Site Contractors directing them to incorporate into their installation procedures, provisions to preclude failure to document incomplete work.

This letter gen-erically identifies the subject violation, the cause and relates the specific violation, as identified, to all contractors by exampling the ineffectiveness of one contractor to document and report a

de-'iciency and the effect it can have on the work of another contractor.
