INSPECTION REPORT NO. 50-244/93-08

This letter refers to the announced

inspection conducted by Mr. H. Gregg of this office on

April 12-16 and May 24-28, 1993, at the R. E. Ginna Nuclear Power Plant;

The inspector

discussed

the results of the inspection with Mr. J. Widay and other members of your staff on

April 16, 1993, and with Mr. T. Schuler and other members of your staff on May 28, 1993.

This inspection focused on two specific service water system problems, failed service water

valves and leakage of underground piping, that were identified by your staff.

This inspection

concluded on June 18, 1993, with a review of the status of your actions to resolve the

findings.

The service water system has an important role in protecting the health and safety of the

public by providing coolant to safety-related

components to enable them to perform their

functions.

The inspection consisted of observations of in-process work, removed, valves and

internals, and condition of the piping. In addition, selected reviews were made of your

service water documentation, piping drawings and maintenance records.

Based on the results of this inspection, certain activities relating to timely identification and

correction of service water valve failures appeared

to be in violation of NRC requirements.

Specifically, two manual locked open service water valves that failed in closed. position with

discs separated

from their stems were not identified until the 1993 outage.

These valves

were in failed closed position for an undetermined time during operation.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement

Actions" (Enforcement Policy), 10 CFR Part 2, Appendix C (1992), an enforcement

conference to discuss the apparent violation is scheduled for July 16, 1993, in the NRC

Region I Office in King of Prussia, Pennsylvania.

This enforcement conference willbe open

to public observation in accordance with the Commission's trial program as discussed in the

enclosed Federal Register Notice (Enclosure 2).

Since an enforcement conference willbe

held, no Notice of Violation is presently being issued for this finding. The purposes of this

conference are to discuss the causes

and safety significance of the apparent violation, to

review your completed short-term actions and your proposed long-term corrective actions, to

provide you the opportunity to point out any errors in our report, and to discuss any other

information that willhelp us determine the appropriate enforcement action in accordance with

the Enforcement Policy. In particular, we expect you to discuss:

(1) the corrective actions

for the failures of the valves when the discs separated

from the stems,

(2) the specific

reasons why corrective actions were not, taken earlier when similar,valve failures occurred,

and (3) the ability of.the safety injection pumps to perform their post-accident mitigation-

function had service water flow to the safety injection pump thrust bearing coolers been

isolated either by failure of valve 4739 which was found to be severely degraded oi when

valve 4739 was closed during a service water pump surveillance.

Additionally, our

inspection report described an underground service water leak that we expect you to discuss

including your planned corrective actions.

You willbe advised by separate

correspondence

of the results of our deliberations on these

matters.

No response regarding the apparent violation is required at this time.
