SUMMARY
REPORT NO. 50-335/87"14 This letter refers to the Enforcement Conference held at our request at the St.
Lucie 'site on August 7, 1987.-
This meeting concerned operation of the-St.
Lucie Unit 1 facility for the period June 8, 1987 to June 19,
- 1987, with the Component Cooling Water Systems cross-connected.
It is our opinion that the meeting was beneficial and provided a better understanding of inspection findings and enforcement issues.
After careful consideration of the information presented at the
- meeting, we conclude that you identified the violation at the first opportunity, the safety significance fits a
Severity Level IV, you promptly reported the
- event, you took prompt and extensive corrective actions to prevent recurrence, and it was not a violation that could reasonably be expected to have been prevented by previous corrective actions.
In order to continue to encourage and support your initiative for self-identification and corrective
- actions, we are electing not to issue a
Notice of Violation for this event.
Our records will be corrected to reflect the withdrawal of Violation 50-335/87-14-01 and Unresolved Item 50-335/87-14-02.
A copy of the meeting summary and a list of attendees is enclosed.
In accordance with Section 2.790 of the NRC's "Rules of Practice,"

Meetin Summar An Enforcement Conference was held at the St.
Lucie site, on August 7, 1987 to discuss the cross-connection of Unit 1
Component Cooling Water Systems (CCW) from June 8,
1987 until June 19, 1987.
Mr. Ernst opened the meeting by expressing the concern that the lack of redundancy existed for 11 days and that approximately 30 shift turnovers on the status of safety related-systems failed to identify the existence of the cross-connection.
Event Discussion The details of the event are documented in Inspection Report 335/87-14 which was transmitted to the licensee on July. 28, 1987.
The licensee acknowledged the violation of redundancy.

- Pearce, FPL, discussed the 'circumstances leading up to the event, cause identification and corrective actions.
On June 6,
1987, the 1B CCW Heat Exchanger was removed from service for leak repair.
The Equipment Clearance, hung on tne CCW Heat Exchanger, did not include the by-pass valves 1-SB-14169 and 1-SB-14439.
When the operator established the by-pass, he elected not to add these valves to the clearance, rather he entered 1-SB-14169 (shown on engineering drawings as locked closed) on the Valve Switch Deviation Log.
Thus when the Heat Exchanger was put back into service on June 8, 1987 only the valves listed on the clearance were independently verified to be in the proper position.
Control room indications of valve position and header flow rate would 'not provide information on the mispositioned manual valves.
On June 19, 1987 during the conduct of routine surveillance on each
The mispositioned valves were identified and repositioned within six minutes.
The licensee's investigation of the root cause included review of past clearances for cleaning CCW Heat Exchangers.(a routine maintenance operation) and determined that for the last five occasions the valves were included on the clearance; therefore, when the system was returned to service, these manual valves were independently verified.
The licensee promptly revised the clearance procedure to require listing of all valves that are repositioned, not only those in the flow path.
The licensee also reviewed the procedure for routine Safety-Related System walkdowns.
The review indicated that, similarly, only valves in the flow path were required to be independently verified.
This procedure was promptly revised to include all valves in the safety system.
The licensee conducted a further review of procedures for the Valve Switch Deviation Log
.and found that this log was being reviewed by operations
quarterly.
This procedure was revised to include daily review on the midnight shi ft.
To assure prevention of the initial event, the licensee discussed the issue with individuals involved and conducted formal training for all licensed operators.
An engineering review of the significance of the event was discussed by T. J.
- Vogan, FPL.
The system as normally operated is in a
by-pass configuration even with the manual valves closed.
Redundancy is provided by automatic closure signals to motor operated valves.
To estimate the significance of the event, the licensee used a simplified system and calculated the system failure probabilities with the valves open vs.
- closed, The results showed minimum effect on the accident failure probability for these passive type failures.
The licensee compared these with an active type failure permitted by Technical Specification LCO for one train being inoperable.
These latter results indicated a
probability of failure almost two orders of magnitude greater than the event condition.
In addition the licensee stated that although redundancy was lost there was no loss of operability of either CCW.
For completeness, the licensee evaluated the consequences of loss of the A and 8
CCM Headers.
The components serviced by the Headers include Mechanical Seals for HPSI, LPSI and Containment Spray
- Pumps, Containment Coolers and Shutdown Cooling Heat Exchanger.
Under accident conditions coolant to the pump mechanical seals is not imperative, the Containment Coolers are backed up by 2, each 100 percent, capacity spray headers, and coolant to the shutdown cooling heat exchanger could be restored before there would be a need.
Mr. Ernst thanked the licensee for a professional and technical discussion of the issues related to this event.
