Finding:
Technical Specification 6.8.l.c requires that written procedures
shall
be
established,
implemented
and maintained covering surveillance
and test
activities of safety-related
equipment.
Contrary to the above:
a.
Maintenance
Procedure
M-0017, Revision 2,
was inadequately
established
and
implemented,
in that the procedure failed to direct
the mechanic
to ensure
the nozzle ring set-screw
was installed in
a
notch vice on
a land and the mechanic
turned the nozzle ring the
wrong direction after
checking the relief point.
Improper set
screw
installation is credited
as
a major factor in causing early relief
valve operation
on December
19,
1981.
Response:
1.
FPL concurs with the finding.
2.
The procedure
was not clear
on the set
screw installation.
3.
As corrective action, the replacement relief valve was checked to
assure
correct installation.
4.
As corrective action to prevent future problems,
procedure
M-0017 was
revised to clarify the nozzle ring and set
screw installation
and
this area
was emphasized
by maki ng this procedural
step
a
"gC Hold
Point".- Additionally, the training
program is stressing
the
importance of correct nozzle
and adjusting ring installation.
5.
Full compliance
was achieved
on March 2, 1982.
