St. Lucie Units 1 & 2 Docket Nos. 50-335 and 50-389 IE Inspection Reports 81-35 and 81-26 Finding:

Technical Specification 6.8.1.c requires that written procedures shall be established, implemented and maintained covering surveillance and test activities of safety-related equipment.

Contrary to the above:

a.

Maintenance Procedure M-0017, Revision 2, was inadequately established and implemented, in that the procedure failed to direct the mechanic to ensure the nozzle ring set-screw was installed in a notch vice on a land and the mechanic turned the nozzle ring the wrong direction after checking the relief point.

Improper set screw installation is credited as a major factor in causing early relief valve operation on December 19, 1981.

Response

1.

FPL concurs with the finding.

2.

The procedure was not clear on the set screw installation.

3.

As corrective action, the replacenent relief valve was checked to assure correct installation.

4.

As corrective action to prevent future problems, procedure M-0017 was revised to clarify the nozzle ring and set screw installation and this area was emphasized by making this procedural step a "QC Hold Point". Additionally, the training program is stressing the importance of correct nozzle and adjusting ring installation.

5.

Full compliance was achieved on March 2,1982.
