Attachment A

RII:JRH 50-389/78-12 CPL:QI 10.71, Field Inspection of Structural Concrete, requires that the first batch produced each day and every 50 cubic yards per mix type be sampled and tested for air temperature and slump.

Contrary to this, quality records for concrete placement in areas 21 and 22 of 'the Reactor Auxiliary Building show that 5 of the 23 required sampling intervals varied from 60 to 80 cubic yards.

We agree with your finding as stated.

An error was made in not sampling every 50th yard.

Our review of the situation indicates that our samples taken were not taken from the truck representing the next consecutive 50th cubic yazd.

We have reviewed" the test data from samples taken and have found that the correct number of samples were taken and that the test results were within specified tolerances.

s Our'QC placement point inspectors have been instructed to maintain in-process tests as close as possible to consecutive 50 yard intervals.

In-creased coordination between the Batch Plant and placement test points will be implemented on all future placements.
