APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company License No.
CPPR-144 Based on the results of the NRC inspection conducted on November 28 through December 1,
1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in our correspon-dence to you dated December 31, 1974.
Criterion V of Appendix B, 10 CFR 50, as implemented by PSAR Section 17.1A.5, requires in part that activities affecting quality are prescribed and to be accomplished in accordance with documented procedures.
Procedure QI 10.71, Field Inspection of Structural
- Concrete, requires that the first batch produced each day and every 50 cubic yards per mix type be sampled and tested for air temperature and slump.
Contrary to the above quality records for concrete placement in areas 21 and 22 of the Reactor Auxiliary building show that five of the 23 required sampling intervals varied from 60 to 80 cubic yards'his is a deficiency.
